2. Collect your documentation and either attach it to the intake form or drop it off at .
3. Attend an intake appointment to discuss classes, the impact of your disability, and possible accommodations with our staff.

1. Inform your instructor which testing accommodations you鈥檇 like to use.
2. Ask the instructor to send the appropriate office (Disability Services or Testing Services) the test proctoring form. For assistance in determining which office you should schedule with, please view the Accommodated Test Scheduling Guide.
3. Schedule your exam with the appropriate office.*
4. Request your Letter of Accommodation and discuss the accommodations you will use with each instructor.

*When scheduling, include the name of your course, the date and time you鈥檇 like to test, and the accommodations you are requesting. Please bring a photo ID with your first and last name to your test appointment.

Note: 91暗网 has a process to ensure equal treatment for pregnancy-related conditions. Students with pregnancy-related conditions, like any other students requesting classroom accommodations, are entitled to reasonable accommodations.
