2. Collect your documentation and either attach it to the intake form or drop it off at .
3. Attend an intake appointment to discuss classes, the impact of disability and possible accommodations with our staff.

1. Inform your instructor which testing accommodations you'd like to use.
   
2. Ask the instructor to send the appropriate office (Disability Services or Testing Services) the test proctoring form. For assistance in determining which office you should schedule in, please view the Accommodated Test Scheduling Guide.
   
3. Schedule exam with appropriate office.*
4. Request your Letter of Accommodation and discuss the accommodations you will use with each instructor.
   

*When scheduling, include the name of your course, date and time you'd like to test and the accommodations you are requesting to use. Please bring a photo ID with first and last name to your test appointment.
